Urgent concern? If someone is seriously unwell or deteriorating after a head injury, call 000 now — don't wait for a screening result.
Screening is not a diagnosis or clearance. If concussion is suspected, remove the athlete from play and arrange appropriate medical assessment. Do not return to sport on the same day.
What happens to my answers?
Your answers are processed on this device. MHCG does not receive your responses through this app. You can save or print a report yourself. You do not need to give your name, date of birth or contact details. Only the chosen age group is passed to Return to Play if you tap Continue; the Return to Play tool asks for exact age and injury date to build a safe plan.
Step 1 of 5
Select an age group
That's all we need to begin. No name, email, or personal details required.
For a child aged 8 to 12, this screening should be completed by a parent or caregiver, answering about the child.

A quick orientation check
If this happened during sport and the athlete is awake, alert and safe to question, ask the following. An incorrect or uncertain answer is a reason to suspect concussion and arrange medical assessment. Correct answers do not rule concussion out.
Do not use this check for emergencies or to decide whether someone can return to play. If they're confused or worsening, stop and seek urgent medical assistance.
One or more answers were incorrect or uncertain. Keep the athlete out of sport and arrange an appropriate medical assessment. This is not a diagnosis.
Please answer each question, or choose Skip.
This is an original MHCG sporting-context question screen informed by published concussion guidance, not a SCAT6 administration or a standalone clearance test. Do not infer clearance from a normal score.

Rate current symptoms
Rate each symptom below. Scroll through the full list, then continue once at the end.
Please rate every symptom before continuing.

Step 5 of 5
No symptoms reported
Severity bands are MHCG triage guides to help you decide how quickly to seek review. They are not validated diagnostic thresholds.
This report summarises responses to a concussion screening questionnaire. It supports, but does not replace, assessment by a qualified healthcare professional.
Screening cannot rule out concussion. If concussion is suspected after a knock, remove the athlete from play and arrange appropriate medical assessment; do not return to sport on the same day. A symptom score of zero is not clearance.
Recommended next steps
🧠 Relative rest
Reduce activities that clearly worsen symptoms during the first 24 to 48 hours. Light daily activity is generally encouraged where tolerated.
🏃 Return to activity
Gradually increase cognitive and physical activity. Avoid contact sport or activities with a risk of another impact until assessed and cleared. Track your staged recovery with the MHCG Return to Play Guide.
👁 Monitor symptoms
Seek further review if symptoms worsen, new symptoms appear or daily activities become difficult.
🏥 Arrange assessment
A clinical assessment should consider the injury, neurological function, balance, eye movements, cognition and exercise tolerance.
🚨 Know the red flags
Call 000 for a severe or worsening headache, vomiting, seizure, increasing confusion, weakness or difficulty waking.
📄 Keep the report
Save or print the full report to share with your GP, school, workplace or sporting club.
Your next steps
What happens after screening?
If concussion is suspected, remove the athlete from play and arrange an appropriate medical assessment. MHCG can support ongoing recovery with a virtual consultation or an appointment at our Williamstown clinic. This screening does not diagnose concussion or clear an athlete to return to sport.
The free Return to Play guide complements appropriate clinical care. Only your selected age group carries to the next tool on this device. You'll enter the exact age and injury date when starting a recovery plan; no screening answers or symptom scores transfer. Screening does not provide medical clearance.

Please get help now
The warning sign selected may indicate a more serious brain or spinal injury.
Call triple zero (000) now and ask for an ambulance, or have someone take you to the nearest emergency department. Do not drive yourself.
On a computer? Use any phone to dial triple zero (000). If you are deaf or have a hearing or speech impairment, call 106 via the National Relay Service. If you're unsure, call anyway. It is always better to be checked.
Concussion screening checklists are only designed for ages 8 and over. A child under 8 who has had a knock to the head should be assessed in person by a health professional.
Call 000 immediately if the child has any of the following: loss of consciousness, vomiting, a seizure, increasing drowsiness or difficulty waking, weakness or numbness, neck pain or tenderness, a severe or worsening headache, increasing confusion or unusual behaviour, or a visible change in head shape.
If none of these apply, arrange a same-day review with your GP, or contact us. We can advise on the right next step for young children.
